These run separately. Entering on day 89 of validity still gives you the full 30-day stay. Applying more than three months before travel is counterproductive, since the visa can expire before you fly.
Extensions
The Umrah visa is not routinely extendable. Where an extension is granted it is for a documented medical reason, applied for through Nusuk before expiry.
There is no visa run. It is single entry, leaving and re-entering requires a new visa.
Overstaying
A fine, deportation at your own cost, and a re-entry ban that typically runs several years. It is treated seriously and it is not negotiable at the airport.
